Description
Arising along the lateral aspect of the female breast, the lateral mammary arterial component courses from the axillary region toward the glandular parenchyma, sending branching arterioles medially into the lobules embedded within adipose tissue. Deep to the breast, the pectoralis major muscle forms a red muscular bed against the chest wall, while fibrous connective septa (suspensory ligaments) traverse from superficial fascia toward the deeper tissues, compartmentalizing fat and gland. Fine blue sensory nerves run with the vascular network, tracking between lobules and across the lateral quadrant toward the nipple areolar complex. Spatially, the arterial branches lie superficial to pectoralis major but deep to the most superficial fatty layer, with distal twigs insinuating between glandular lobules. Lateral breast vascular anatomy matters when planning surgery that approaches the axilla. During sentinel lymph node biopsy, axillary dissection, or lateral oncoplastic incisions, injury to the lateral thoracic artery and its mammary branches can drive troublesome bleeding and compromise perfusion to skin flaps, a known contributor to delayed healing or necrosis after mastectomy. This view also supports teaching of why the lateral quadrants often drain to axillary nodes and why neurovascular bundles in the lateral breast can influence postoperative sensation.
