Description
Running within the hepatic parenchyma, the middle hepatic vein courses superiorly toward the suprahepatic inferior vena cava, lying in the principal plane between the right and left functional lobes. Tributary venules converge from the adjacent segments (classically segments IV, V, and VIII), forming a progressively larger-caliber vessel as it approaches its terminal confluence. Blue venous channels are contrasted against the surrounding liver tissue, with portal venous branches also present deeper in the parenchyma and arranged in a branching, segmental pattern. Appreciating the middle hepatic vein’s trajectory matters when you plan a resection along Cantlie’s line, since this vein often defines the intersegmental plane and can be encountered unexpectedly during a right anterior sectionectomy or a left medial (segment IV) resection. Small variations in size and the exact drainage territory influence venous congestion risk after hepatectomy, and they change where surgeons can safely divide parenchyma without compromising outflow.
