Description
Running within the main lobar fissure, the middle hepatic vein courses superiorly from the central hepatic parenchyma toward the hepatocaval junction, lying between the left and right functional hemilivers as defined by Couinaud. An anterior perspective places it deep to the anterior liver surface, with its tributaries draining adjacent segments and converging toward the superior aspect of the organ. Superiorly, the vessel is expected to approach the inferior vena cava just inferior to the diaphragm. It is a midline landmark. Surgeons and interventional radiologists track the middle hepatic vein because it maps the plane of a central hepatectomy and helps distinguish segment IV from segments V and VIII, a distinction that matters when planning resection margins and preserving venous outflow. Variations are common, including shared trunks with the left hepatic vein, and these patterns influence bleeding risk and postoperative congestion. In living-donor liver transplantation, the decision to reconstruct middle hepatic vein tributaries, such as those draining segments V and VIII, often determines anterior sector viability.
