Description
Running vertically along the lateral pericardium, the pericardiophrenic vein is shown in anterior perspective as it descends with the phrenic nerve in the fibrous pericardium, positioned anterior to the lung root and medial to the mediastinal pleura. Small venules from the pericardium and adjacent mediastinal tissues converge into the main channel as it courses inferiorly toward the diaphragm. Superiorly, the vessel is oriented toward its usual termination in the internal thoracic vein and then the brachiocephalic venous system, in close relationship to the superior vena cava and the anterior surface of the heart. Coronary arteries and the great vessels provide deep context while remaining secondary to the pericardiophrenic pathway. Clinically, this venous route matters because it mirrors the phrenic nerve’s course and frequently enters the operative field during pericardial windows, minimally invasive thoracic procedures, and anterior mediastinal dissection. Injury can produce brisk bleeding against a confined pericardial surface, and its proximity to the phrenic nerve raises the stakes for nerve preservation when mobilizing pericardium or controlling pericardial effusions. A common teaching point. The vein also becomes a useful landmark when tracing anterior mediastinal drainage patterns and collateral flow pathways in central venous obstruction involving the brachiocephalic veins or superior vena cava.
