Description
Running along the anteromedial thigh, the femoral vein ascends from the adductor canal to pass deep to the inguinal ligament, continuing proximally as the external iliac vein. Medial to it lies the femoral canal and femoral lymphatics, while the femoral artery sits lateral within the femoral sheath, a relationship that stays consistent as the vessels track superiorly toward the femoral triangle. Superficial to the sheath, the great saphenous vein can be inferred as it approaches the saphenofemoral junction to drain into the femoral vein. Bones of the pelvis, femur, patella, tibia, fibula, and foot provide firm landmarks for orientation. Clear geography. Clinically, the anterior presentation in a male is practical for mapping access and compressibility in the groin: the femoral vein is the intended target for central venous cannulation when upper-body sites are not available, and it is also the structure at risk for inadvertent venous puncture during femoral arterial catheterization. Its deep location within the femoral sheath explains why femoral deep vein thrombosis can propagate proximally with embolic potential, while superficial thrombophlebitis of the great saphenous becomes more concerning as it approaches the saphenofemoral junction. The juxtaposition of artery and vein also teaches why hematoma, pseudoaneurysm, and arteriovenous fistula cluster at this site after percutaneous procedures. Suitable for gross anatomy teaching in lower limb blocks, vascular anatomy modules, and surgical or interventional radiology atlases that need an uncluttered anterior roadmap of the femoral triangle and proximal thigh vessels. It also supports patient-facing education on DVT location and groin line placement by pairing vessel course with familiar bony landmarks.
