Description
Arising within the hepatic parenchyma and coursing superiorly toward the diaphragmatic surface, the middle hepatic vein (vena hepatica media) is presented in anterior orientation as it converges on the inferior vena cava. Its main trunk runs in the principal plane of the liver (Cantlie line), separating the right and left functional lobes, with tributaries draining the medial left lobe (segment IV) and the anterior sector of the right lobe (segments V and VIII). Superiorly and posteriorly, the vessel approaches the caval groove where the hepatic veins enter the intrahepatic segment of the IVC just inferior to the diaphragm. Smaller venous radicles are visible radiating peripherally within the semi-transparent liver tissue. A clean central landmark. Surgical and interventional planning often depends on recognizing the middle hepatic vein as a boundary rather than just a drain. During right hepatectomy, extended left hepatectomy, or split-liver transplantation, preserving or reconstructing venous outflow from segments V and VIII can prevent postoperative congestion, and this is the vein that dictates those outflow territories. The anterior view also supports correlation with venous phase CT or MR, where the middle hepatic vein is used to map Couinaud segments and distinguish functional anatomy from the falciform ligament based “lobes.” Use this asset for hepatobiliary surgery atlases, radiology teaching files on hepatic venous anatomy, and preoperative planning visuals explaining segmental resection lines and outflow preservation.
