Description
Running vertically along the parasternal line, the paired internal thoracic veins ascend on either side of the sternum, receiving anterior intercostal veins from the upper intercostal spaces and small tributaries from the anterior chest wall. Pectoralis major forms the broad anterior muscular plane, with the internal thoracic vessels positioned deep to the costal cartilages and adjacent to the internal thoracic artery, while the rectus abdominis and its sheath lie inferiorly along the midline. Deltoid and biceps brachii frame the superior lateral margins, providing surface landmarks that orient the thoracic inlet region. Clear laterality and depth cues matter here. Clinically, the internal thoracic vein is encountered when raising parasternal flaps, opening the anterior mediastinum, or harvesting the internal thoracic artery for coronary artery bypass grafting, where venous branches can obscure the artery and contribute to troublesome bleeding if not controlled. Its connections into the brachiocephalic vein and anterior intercostal network also make it part of collateral drainage patterns in superior vena cava obstruction and a potential conduit for chest wall varices.
