Internal Thoracic Vein, Anterior View

Description

Running vertically on either side of the sternum, the internal thoracic vein (vena thoracica interna) ascends on the deep surface of the anterior thoracic wall, paired with its comitant artery along the posterior aspect of the costal cartilages. Superiorly it converges toward the thoracic inlet and drains into the brachiocephalic vein near the venous angle, lying medial to the pectoralis major and anterior to the pleura and lungs. Expect accompanying venous tributaries from the anterior intercostal spaces and perforating branches that pass anteriorly toward the pectoral region. Superficial musculature of the male upper trunk, including pectoralis major, deltoid, and rectus abdominis, provides orientation for this deeper parasternal vessel. For chest wall surgery and interventional access, the internal thoracic vessels are more than academic landmarks. Parasternal dissection during internal mammary lymph node biopsy, sternal wiring, or median sternotomy risks injury to the internal thoracic vein, and bleeding can retract into the confined retrosternal space. On imaging, parasternal vessel enlargement can signal collateral flow, for example in superior vena cava obstruction where internal thoracic venous pathways contribute to anterior chest wall collaterals draining toward the brachiocephalic system. Small structure, real consequences. Educators can drop this figure directly into thoracic wall anatomy teaching (gross anatomy, cardiothoracic surgery, or interventional radiology), where learners need to relate parasternal vessels to pectoral landmarks and the brachiocephalic confluence.
