Centered along the internal occipital crest, the occipital sinus appears as a small midline venous channel within the attached margin of the falx cerebelli, immediately posterior to the cerebellar vermis and superior to the foramen magnum. Superiorly it trends toward the confluence of sinuses at the internal occipital protuberance, while inferiorly it communicates with the marginal sinus encircling the foramen magnum and the internal vertebral venous plexus. Adjacent dural venous pathways are typically visible in this region, including the transverse sinuses running laterally from the confluence and the sigmoid sinuses curving inferiorly toward the jugular foramina, with cortical veins and dural arteries color coded on the surface of the cerebrum and posterior fossa. Cranial nerves emerging from the brainstem and the posteriorly placed cerebellum provide orientation in the infratentorial compartment. Small, but anatomically specific. The occipital sinus matters because it sits at the junction of intracranial and vertebral venous drainage, and its caliber and persistence vary more than most learners expect. During posterior fossa exposure and midline suboccipital craniotomy, injury to the occipital or marginal sinus can produce brisk bleeding in a narrow corridor, and preoperative awareness of venous variants helps explain unexpected blood loss or limitations in dural opening. In dural arteriovenous fistula workups, understanding its connections to the confluence and vertebral plexus clarifies potential retrograde venous routes and symptom patterns. Use this asset in neuroanatomy teaching on dural folds and dural venous sinuses, in neurosurgical atlases illustrating the suboccipital approach, or in radiology and neurointerventional materials that correlate MRV/CTV with posterior fossa venous anatomy and variants.
