Description
Running superficially along the anterior forearm, the palmaris longus lies in the intermediate layer of the flexor compartment, positioned between flexor carpi radialis laterally and flexor carpi ulnaris medially. Its fusiform muscle belly arises from the medial epicondyle of the humerus via the common flexor tendon, then transitions distally into a slender tendon that passes superficial to the flexor retinaculum to blend with the palmar aponeurosis. A full anterior male figure context also places it in relation to neighboring superficial flexors, including pronator teres proximally and flexor digitorum superficialis deeper and slightly lateral, with the carpal region and metacarpals providing bony landmarks. Clinically, palmaris longus matters out of proportion to its modest function, because its tendon is frequently harvested for grafting in hand and reconstructive surgery, including tendon transfers and ligament reconstructions. Variability is the rule, congenital absence is common, and accessory or reversed muscle bellies can contribute to anterior wrist or forearm symptoms by crowding the median nerve region near the carpal tunnel and proximal palm. Surface anatomy teaching often uses it as a quick landmark: with wrist flexion and thumb to little finger opposition, the tendon can stand out just ulnar to flexor carpi radialis. Use this illustration in gross anatomy and kinesiology coursework to orient the palmaris longus within the broader muscular map, then zoom in conceptually to the forearm flexor compartment and palmar aponeurosis.
