Description
Centered in the posterior abdominal wall, the slender psoas minor lies on the anterior surface of the psoas major, running inferiorly from the bodies and intervertebral disc of T12 to L1 toward the pelvic brim in a female trunk. Its long tendon crosses anterior to the iliacus and the iliopsoas compartment to blend with the iliac fascia and attach near the iliopectineal eminence and pectineal line of the pubis, just medial to the iliopubic ramus. Lateral to this straplike muscle, the broader psoas major descends beside the lumbar vertebrae, joining the iliacus in the iliac fossa to form the iliopsoas that passes deep to the inguinal ligament toward the lesser trochanter of the femur. Bony landmarks anchor the relationships. Psoas minor matters because it is variable, absent in a substantial portion of individuals, and its presence can confuse identification of the iliopsoas tendon and adjacent retroperitoneal planes during dissection or surgery. Surgeons working through an anterior retroperitoneal approach to the lumbar spine, or exposing the pelvic brim for gynecologic oncology, often use the psoas major as a landmark while protecting the lumbar plexus embedded within it, and the psoas minor tendon can be mistaken for fascia or a fibrous band. When present and tight, it may contribute to anterior pelvic tilt and localized lumbar discomfort, and it can be encountered near inflammatory processes such as iliopsoas bursitis or a psoas abscess tracking along the fascial compartment.
