Description
Angling onto the right and inferior aspect of the heart, the right coronary artery (RCA) tracks in the right atrioventricular (coronary) sulcus between the right atrium superiorly and the right ventricle inferiorly. Distally, the right inferolateral branch courses obliquely across the diaphragmatic surface of the right ventricle toward the acute (inferior) margin, giving smaller myocardial branches into the right ventricular free wall. Nearby epicardial landmarks you would expect in this projection include the right ventricular outflow tract anteriorly, the inferior vena cava entry into the right atrium posteriorly and superiorly, and the coronary sinus in the posterior atrioventricular groove. This branch matters because it represents a common distribution of RCA perfusion to the inferolateral right ventricle, a territory implicated in inferior myocardial infarction patterns and in right ventricular ischemia when a proximal RCA lesion limits distal flow. Interventional cardiology and cardiac surgery both rely on this surface anatomy: the RCA and its inferolateral branches run in fat within the atrioventricular groove and can be at risk during tricuspid valve surgery, right atriotomy, or epicardial dissection near the acute margin. Dominance also matters. In right-dominant hearts, distal RCA supply relates closely to the posterior interventricular region, affecting infarct extent and conduction system ischemia.
