Description
Arising from the right coronary artery in the right atrioventricular (coronary) sulcus, the intermediate atrial branch courses superiorly and slightly posteriorly toward the right atrium. Along its path, it runs medial to the right atrial appendage and approaches the interatrial groove near the junction of the superior vena cava and right atrium. The proximal right coronary artery, aortic root, and the anterior contours of the right ventricle provide the main spatial landmarks, with companion cardiac veins shown in blue tracking parallel venous return. For teaching coronary topography, this vessel matters because it sits at the crossroads between epicardial arterial supply and the atrial conduction substrate. Proximity to the sinoatrial node region means a prominent atrial branch from the right coronary artery can be implicated in ischemic sinus node dysfunction and atrial arrhythmias during proximal RCA occlusion, and it is a practical landmark when interpreting CT coronary angiography or catheter angiograms where small atrial branches can be overlooked. Variant dominance patterns and atrial branch caliber also influence how much atrial myocardium becomes vulnerable during right-sided infarction.
