Description
Presented in anterior orientation, the right coronary artery courses in the right atrioventricular (coronary) sulcus between the right atrium superiorly and the right ventricle inferiorly, giving off the sinuatrial nodal branch proximally as it turns toward the superior right atrium. That nodal artery ascends cranially and medially toward the junction of the superior vena cava and right atrium, tracking toward the sinoatrial node at the superior end of the sulcus terminalis. The ascending aorta and pulmonary trunk sit superior and central, framing the coronary origins at the aortic root, while superficial cardiac veins accompany the arterial pattern in the epicardial fat. Identification of the sinuatrial nodal branch matters because right coronary dominance commonly supplies the sinoatrial node, and compromise of this vessel can translate into symptomatic sinus node dysfunction. Proximal right coronary artery occlusion in an inferior myocardial infarction can involve the nodal artery and present with sinus bradycardia, sinus arrest, or sinoatrial exit block. Electrophysiology and cardiac surgery also care about this course: cannulation or atrial incisions near the superior cavoatrial junction risk regional ischemia when the nodal artery runs close to the right atrial epicardium.
