Description
Seen from the inferior (diaphragmatic) surface of the heart, the right coronary artery courses in the right atrioventricular (coronary) sulcus and gives off the right inferolateral branch as it sweeps posteriorly toward the crux. The highlighted vessel tracks obliquely across the inferior wall of the right ventricle, running lateral to the posterior interventricular sulcus and its posterior descending artery (posterior interventricular branch) when present. Nearby surface landmarks include the inferior aspects of the right atrium and right ventricle, the left ventricle forming the leftward diaphragmatic contour, and the bases of the great vessels superiorly at the atrial level. For coronary dominance assessment, this branch matters because it marks how much of the diaphragmatic myocardium the right coronary system supplies, and it helps you anticipate territory at risk in right coronary artery occlusion. Ischemia or infarction involving the inferolateral branches can present with inferior wall ECG changes and may extend toward the posterolateral left ventricular region in right-dominant or co-dominant patterns. Interventional planning also hinges on this anatomy: guiding catheter engagement of the right coronary ostium and wire passage into posterolateral branches can be challenging when the right coronary artery is tortuous along the atrioventricular groove. Cardiology and anatomy faculty can drop this inferior view directly into lectures on coronary circulation, dominance, and diaphragmatic surface landmarks, or into board-style teaching on localization of myocardial infarction. It also fits well in surgical and cath lab education materials that map right coronary branching to revascularization targets on coronary angiography or CT coronary angiography.
