Description
Seen in lateral profile, the distal tibia and fibula descend to the ankle mortise, where the talus sits superior to the calcaneus and transitions anteriorly into the midfoot. The calcaneus forms the posterior heel, while the cuboid lies anterior to it on the lateral column and the navicular and cuneiforms are suggested more medially by the alignment of the metatarsal bases. Five metatarsals project distally toward the toes, ending in the phalanges, with the hallux showing its characteristic two-phalange configuration and the lesser toes showing three. Skin and hair of the African adult male foot and distal leg remain visible beneath the skeletal overlay, anchoring bony landmarks to surface anatomy. Lateral views of the foot skeleton are the workhorse for teaching arches and load transfer because they let you track the calcaneal tuberosity, subtalar region, and the longitudinal arch in one uninterrupted line from hindfoot to forefoot. That matters in common clinical problems: calcaneal fractures from axial load, talar neck fractures with risk to the talar blood supply, and lateral column pain after cuboid impaction in inversion injuries. A clear side projection also supports discussion of plantar fasciitis and enthesopathy at the calcaneal tubercle, where patients localize tenderness just anterior to the heel pad. Bones dictate the exam. Use this asset in gross anatomy and musculoskeletal medicine courses to correlate palpation points (lateral malleolus, calcaneus, fifth metatarsal base) with the underlying tarsal and metatarsal framework, or in orthopedics and podiatry texts when introducing hindfoot alignment, fracture patterns, and basic radiographic orientation.
