Description
Visible within the posterior globe are the vorticose (vortex) veins as large-caliber choroidal outflow channels, collecting blood from smaller choroidal venules and converging toward their ampullae before piercing the sclera. Radial tributaries from the choroidal vascular bed course posterior to the retina and deep to the sclera, arranged in a spoke-like pattern that trends from the posterior pole toward the equatorial region. Anteriorly, the cornea, iris, and pupil establish the ocular orientation, while the optic nerve exits the posteromedial aspect of the globe as the central landmark for the posterior segment vasculature. Fine vessels within the choroid sit external to the retinal layers. Vorticose veins matter because they represent the dominant venous drainage pathway of the choroid and can become a limiting factor when venous outflow is impaired. Congestion at or near the scleral exit points is implicated in disorders such as uveal effusion syndrome and can be accentuated by scleral thickening, where impaired transscleral drainage and vortex vein compression contribute to choroidal detachment and serous retinal detachment. This perspective also supports teaching of posterior segment anatomy by separating the choroidal circulation (high-flow, pigmented layer) from the retinal circulation, a distinction that guides interpretation of indocyanine green angiography and choroidal OCT findings. Use this artwork in ophthalmology and optometry curricula when introducing the uvea, choroid, and venous outflow anatomy, or in surgical atlases discussing scleral windows, posterior sclerotomy planning, and procedures that risk vortex vein injury and suprachoroidal hemorrhage.
