Description
Curving across the posterior half of the globe, the vorticose veins (venae vorticosae) collect a dense plexus of choroidal venules and coalesce into several large trunks that pierce the sclera in the equatorial region. Anteriorly, the cornea, anterior chamber, iris, and pupil sit in front of the crystalline lens, while posteriorly the retina lines the inner wall, separated from the sclera by the vascular choroid where most of these venous tributaries arise. Nasal and temporal drainage territories can be inferred as the tributaries sweep laterally and medially toward their nearest vortex vein ampulla. Fine vessels converge. It is orderly, not random. Attention to the vortex vein system matters because it is the dominant outflow pathway for the choroid and a surgical landmark in the posterior segment. Transscleral procedures near the equator, including scleral buckle placement or choroidal drainage for suprachoroidal hemorrhage, risk injuring a vortex vein and causing brisk bleeding or postoperative choroidal effusion. This venous anatomy also anchors teaching around uveal congestion and the hemodynamic changes seen in conditions such as uveal effusion syndrome, where impaired vortex vein outflow is one proposed mechanism.
