Description
Centered within the male thorax, the cardiac silhouette sits slightly left of the midline, with the apex directed inferiorly and to the left toward the diaphragm. Superiorly, the great vessels arise from the base of the heart, with the ascending aorta and pulmonary trunk projecting toward the mediastinum, and the cardiac ventricles forming most of the anterior (sternocostal) surface. A single lung is rendered lateral to the mediastinum, framing the heart along its medial surface, while a cutaway diaphragm spans the inferior boundary of the thoracic cavity beneath both organs. Anterior orientation is the teaching view most clinicians default to when correlating surface anatomy with chest radiographs, ECG lead placement, and emergency access to the thorax. The proximity of the inferior (diaphragmatic) surface of the heart to the central tendon helps explain why pericardial effusions can track inferiorly and why subxiphoid pericardiocentesis targets the pericardial space by skirting the diaphragm and liver. You also get a clear sense of why left lower-lobe processes and pleural pathology can mimic cardiac symptoms, given the intimate cardiopulmonary contact along the mediastinal pleura.
