Description
Centered in the anterior thorax, the heart sits within the mediastinum between the right and left lungs, with the right ventricle forming much of the anterior (sternocostal) surface and the left ventricle contributing to the left cardiac border and apex. Superiorly, the ascending aorta and pulmonary trunk emerge from the base of the heart, then diverge into the aortic arch and right and left pulmonary arteries, while the superior vena cava courses down the right side to the right atrium. Each lung flanks the pericardium laterally, and the apices extend superior to the first rib and clavicle into the root of the neck. Trachea and proximal bronchi occupy the superior midline. Lymphatic vessels and nodes track along the great vessels and around the tracheobronchial region. Clear landmarks. Clinically, this anterior arrangement explains why the right ventricle is the chamber most exposed in penetrating anterior chest trauma and why an enlarged pulmonary trunk can alter the left mediastinal contour on a frontal chest radiograph. The depiction of the lung apex above the clavicle anchors surface anatomy for apical pleural injury and pneumothorax after subclavian venous access, and it also frames Pancoast tumor extension into the thoracic inlet.
