Description
Anterior thoracic anatomy is presented with the heart occupying the middle mediastinum and the lungs flanking it laterally. The cardiac apex points inferolaterally to the left, with the right ventricle forming much of the anterior (sternocostal) surface and the left ventricle contributing to the left cardiac border. Along the left lung, the deep cardiac notch indents the anterior margin, while the pericardial silhouette lies medial to the lung roots; inferiorly, the domed diaphragm forms the floor of the thoracic cavity, its muscular fibers curving superiorly toward a central tendon. That left-sided cardiac notch is more than a surface feature, it explains why the pericardium can approach the anterior chest wall with less intervening lung and why pericardial effusions can be assessed and drained using subxiphoid or left parasternal approaches. This perspective also supports teaching of mediastinal compartment anatomy, where the relationship of the heart to the pleural cavities and diaphragmatic domes underpins interpretation of frontal chest radiographs and the expected contours of cardiomegaly, left lower lobe atelectasis, or hyperinflation in obstructive lung disease.
