Description
Oriented in lateral profile, the atlas (C1) is presented as a ring-like vertebra without a vertebral body, with the anterior arch positioned anterior to the vertebral foramen and the posterior arch forming the posterior margin. The bulky lateral masses sit between these arches and carry the superior articular facets, which face superiorly and medially to receive the occipital condyles at the atlanto-occipital joints. Inferiorly, the inferior articular facets of C1 articulate with the superior articular facets of the axis (C2), creating the paired atlanto-axial joints. A short, stout transverse process projects laterally, marking the expected location of the foramen transversarium. This side view matters because it clarifies how load from the skull transfers through the lateral masses rather than a vertebral body, a point that underlies the biomechanics of Jefferson (burst) fractures and their association with axial loading injuries. The relationship between C1 and C2 is also easier to conceptualize here, where subtle malalignment can be appreciated in atlanto-occipital dissociation and in atlanto-axial instability, including rheumatoid ligamentous laxity. Small changes in facet congruence at this level have outsized clinical consequences.
