Description
Sectioned in the median plane, the calvaria and cranial base frame the encephalon, with the cerebrum superior and anterior to the cerebellum and the brainstem descending to become the spinal cord at the foramen magnum. Midline intracranial landmarks such as the corpus callosum arching over the ventricular system are apparent, while the pons and medulla lie anterior to the fourth ventricle and continuous with the cervical cord. Inferiorly, the vertebral column encloses the cord within the vertebral canal, and the normal cervical and lumbar lordoses alternate with thoracic kyphosis along the backbone. Intervertebral discs separate adjacent vertebral bodies, and segmental spinal nerves exit laterally from the cord. A sagittal section is the working view for teaching how the central nervous system is suspended and protected, and it clarifies relationships that get lost in surface renderings, like the tight fit of the brainstem against the clivus and upper cervical spine. Clinically, this alignment underpins MRI interpretation in Chiari I malformation, where cerebellar tonsillar herniation through the foramen magnum can crowd the cervicomedullary junction and impair CSF flow. It also supports discussion of traumatic atlanto-occipital and upper cervical injuries, where small shifts at the craniovertebral junction have outsized neurologic consequences.
