Description
Running in the midline sagittal plane, the section opens the calvaria and vertebral canal to align the cerebrum, cerebellum, brainstem, and spinal cord in one continuous axis. Superiorly, the cerebral hemispheres show cortical gyri and sulci, with the cerebellum positioned posteroinferior to the occipital lobes and the pons and medulla oblongata lying anterior to the fourth ventricle and continuous inferiorly with the cervical spinal cord. Anterior facial skeleton (maxilla and mandible) sits ventral to the cranial cavity, while the cranial base transitions posteriorly into the vertebral column, where individual vertebrae stack from cervical through coccygeal levels. Paired spinal nerve roots exit laterally from the cord to the intervertebral foramina. A mid-sagittal cut is the clearest way to teach the craniovertebral junction, where the medulla passes through the foramen magnum and subtle changes in alignment can translate into major neurologic compromise. This view supports clinical discussions of Chiari malformation, basilar invagination, and cervicomedullary compression, and it maps cleanly to MRI sagittal sequences used in workups for syringomyelia or cervical myelopathy. Landmarks such as the posterior fossa contents relative to the occipital bone and upper cervical spine are easy to orient. It matters.
