Description
Rendered as a mid-sagittal (medial) section, the cranium is opened to the cranial cavity with the cerebrum superiorly, the cerebellum posteriorly, and the brainstem descending into the vertebral canal as spinal cord. Inferiorly, the occipital bone and foramen magnum frame the cervicomedullary junction, with the occipital condyles articulating on the superior articular facets of the atlas (C1). The dens of the axis (C2) sits just anterior to the spinal cord, while the anterior and posterior arches of C1 define the ring around it. Cervical, thoracic, lumbar, sacral, and coccygeal segments of the vertebral column align in their physiologic curvatures, and the intervertebral foramina are indicated laterally where segmental spinal nerves exit. That craniovertebral junction is where anatomy stops being abstract and starts guiding decision making: atlanto-occipital alignment and the relationship of the dens to the medulla and upper cervical cord underpin clearance of suspected C1-C2 injury after trauma. Small changes matter. Basilar invagination, atlanto-axial instability (as in rheumatoid arthritis), and odontoid fractures are all interpreted in relation to these same landmarks, and this sagittal cut clarifies why the cervicomedullary transition is vulnerable to compression and why flexion and extension can worsen symptoms.
