Description
Arising from the superior surface of the C2 vertebral body, the odontoid process (dens) projects cranially toward the atlas, with the anterior arch of C1 lying anterior to it and the spinal canal and posterior elements positioned posteriorly. In lateral profile, the axis contributes a stout vertebral body, a short pedicle, and a posteriorly directed lamina converging toward the spinous process, while the superior and inferior articular facets align to meet C1 above and C3 below. The broader context of the lateral spinal column is also apparent, with cervical lordosis transitioning into thoracic kyphosis and lumbar lordosis, and intervertebral discs interposed between adjacent vertebral bodies. Lateral visualization of C2 matters because the dens is the mechanical keystone for atlantoaxial rotation, and small changes in its alignment can narrow the canal at a neurologically unforgiving level. Odontoid fractures (Type I to III) and traumatic atlantoaxial instability are assessed by scrutinizing dens position relative to the anterior arch of C1 and the posterior vertebral line, and this orientation also supports teaching of the atlantodental interval and retro-odontoid space. Surgical planning for C1-C2 fixation, including transarticular screws or C2 pedicle/pars trajectories, starts with a clear mental model of the axis in profile. A tight corridor.
