Description
Running obliquely from the anterolateral distal tibia (tibial incisura) to the anterior margin of the lateral malleolus, the anterior tibiofibular ligament spans the distal tibiofibular syndesmosis in a lateral view of the male ankle. The fibula sits lateral and slightly posterior to the tibia, forming the ankle mortise superior to the talar trochlea, while the ligament lies anterior to the syndesmotic joint line and superficial to the distal tibiofibular articulation. Inferiorly, the talus caps the calcaneus at the subtalar joint, and the lateral column continues forward through the cuboid to the fourth and fifth metatarsals and phalanges. Clear bony context is maintained. An anterior inferior tibiofibular ligament (AITFL) injury is a hallmark of high ankle sprain, where external rotation of the talus forces the distal fibula laterally and widens the mortise. This lateral perspective clarifies why even small syndesmotic diastasis can destabilize tibiotalar congruence and prolong return to sport compared with isolated anterior talofibular ligament sprains. It also maps directly to the incision planning and fixation corridor for syndesmotic screws or suture-button constructs, placed across the tibia and fibula proximal to the ankle joint line.
