Description
Arising from the transverse processes of the upper thoracic vertebrae and the articular processes of the lower cervical vertebrae, the semispinalis capitis (musculus semispinalis capitis) is presented from a lateral perspective along the posterolateral neck. Its fibers course superiorly and medially to insert between the superior and inferior nuchal lines of the occipital bone, deep to the splenius capitis and trapezius, and posterior to the sternocleidomastoid near the mastoid region. Lateral bony landmarks include the mandible, the occiput, the cervical vertebral column, and the superior thoracic cage with the first rib, clavicle, and a partial scapular outline. Functionally, semispinalis capitis is a prime mover for head and cervical extension and contributes to contralateral rotation when acting unilaterally, making its fiber direction and depth relationship to the transversospinales group a teaching point that students often miss in cadaveric dissection. Headache medicine also cares about this layer. The greater occipital nerve (dorsal ramus of C2) pierces the semispinalis capitis on its way to the scalp, a common site implicated in occipital neuralgia and a useful landmark when planning targeted injections or interpreting posterior neck pain patterns. Course directors in gross anatomy, head and neck anatomy, and physical therapy kinesiology can use this lateral view to clarify which posterior cervical muscles attach to the occipital bone and how they relate to the cervical spinous and transverse processes in profile. It also suits atlas plates and operative anatomy discussions where posterior midline and paramedian approaches to the cervical spine require an accurate mental map of the extensor layers.
