Description
Viewed from the right lateral aspect, the nasal septal cartilage appears as a broad, quadrangular plate forming the anterior and inferior portion of the nasal septum, immediately posterior to the nasal vestibule and anterior to the bony septum. Superiorly it meets the perpendicular plate of the ethmoid, while posteroinferior continuity with the vomer is suggested by its relationship to the vomerine groove and the nasal floor. The cartilage lies medial to the paired nasal bones and maxillary frontal processes, with its anterior margin supporting the columella and the distal dorsum of the external nose. Surrounding craniofacial landmarks, including the orbit, zygomatic bone and arch, maxilla with dentition, and mandible, provide scale and surgical orientation. A lateral perspective is where septal anatomy becomes clinically legible, because you can track how the cartilaginous septum transitions into the rigid bony septum and how that composite partition sits relative to the nasal dorsum and hard palate. Deviations, fractures, and post-traumatic subluxation at the chondrovomerine and chondroethmoidal junctions explain unilateral obstruction and can complicate septoplasty when the caudal strut is under-corrected or over-resected. This is also the working mental model for closed reduction of nasal fractures and for planning septorhinoplasty maneuvers that preserve dorsal support.
