Description
Seen from a lateral profile of the head, neck, shoulder girdle, and upper thorax, the posterior circumflex humeral vein courses around the surgical neck of the humerus deep to the deltoid. It travels posterior to the humeral head in close company with the posterior circumflex humeral artery, passing through the quadrangular space bordered by teres minor superiorly, teres major inferiorly, the long head of triceps medially, and the humeral shaft laterally. Proximally, its tributaries drain the deltoid and periarticular tissues of the glenohumeral joint, then converge toward the axillary vein anterior and medial to the proximal humerus. Bony landmarks of the scapula, clavicle, ribs, and cervical vertebrae provide orientation for the vessel’s depth and curvature. A lateral perspective matters here because it clarifies what is often hard to communicate in text: how a circumflex vein wraps posteriorly and emerges into the axilla without a straight-line route. This is the corridor where a posterior shoulder dislocation, a fracture at the surgical neck, or aggressive retraction during a deltopectoral or posterior approach can injure the accompanying neurovascular bundle, including the axillary nerve that tracks with the posterior circumflex humeral vessels. Small vessel, big consequence.
