Viewed from the lateral aspect of the male lower leg and hindfoot, the calcaneal (Achilles) tendon runs superficially beneath the skin and subcutaneous tissue as the conjoined distal tendon of gastrocnemius and soleus, then inserts on the posterior calcaneal tuberosity. Proximally, the gastrocnemius belly sits posterior and slightly superior, while the soleus contributes deep to it before narrowing into the tendon; anterior to this posterior compartment, tibialis anterior lies along the anterolateral tibia. Along the lateral leg, fibula and the peroneus (fibularis) longus and brevis course inferiorly toward the lateral malleolus, and the sural nerve tracks posterolateral to the Achilles tendon with the small saphenous vein in the same corridor. Clear landmarks. Clinically, the lateral profile of the Achilles tendon matters because it is the easiest way to appreciate tendon thickening, nodularity, and loss of the normal concavity above the calcaneus that often accompanies midportion tendinopathy or insertional disease at the posterior calcaneus. Surgical planning often hinges on this relationship: a posterolateral approach to Achilles repair or debridement offers access while demanding respect for the sural nerve and small saphenous vein, which sit close enough to be tethered or transected during incision placement and paratenon dissection. The distal insertion region also frames common pathology such as Haglund syndrome with retrocalcaneal bursitis, where irritation concentrates between tendon and superior calcaneus.
