Description

Seen in lateral profile, the triceps surae complex (gastrocnemius and soleus) narrows distally into the calcaneal (Achilles) tendon as it descends posterior to the distal tibia and fibula and superficial to the posterior ankle region. The tendon’s broad distal footprint is shown inserting on the posterior surface of the calcaneus, with the course aligned behind the talocrural joint and superior to the plantar aspect of the heel. Lateral bony landmarks of the leg and foot appear in continuity, including the tibia, fibula, talus, calcaneus, and the lateral column of the tarsals and metatarsals. A clear plane around the tendon suggests its paratenon and the adjacent calcaneal region where the retrocalcaneal bursa typically sits. Lateral visualization of the Achilles tendon matters because this is the most common corridor for clinical examination and for appreciating insertional versus midsubstance pathology. Insertional tendinopathy and retrocalcaneal bursitis localize to the posterior calcaneus and can be correlated to a Haglund deformity, while acute rupture more often occurs 2 to 6 cm proximal to the calcaneal insertion where vascularity is relatively reduced. It is a surgical roadmap. The relationship to the posterior calcaneus and ankle joint line helps orient approaches for debridement, reattachment, and reconstruction, and for avoiding iatrogenic injury to the sural nerve coursing posterior to the lateral malleolus.
