Description
Seen from the lateral aspect, the Achilles tendon (tendo calcaneus) descends along the posterior ankle to insert on the posterior surface of the calcaneal tuberosity, immediately superficial to Kager’s fat pad. Inferior to the tendon, the calcaneus and talus define the subtalar region, with the lateral malleolus of the fibula positioned anterior and superior to the tendon’s distal insertion. Distally, the lateral column of the foot is laid out in sequence, calcaneus to cuboid to the fourth and fifth metatarsals, with phalanges extending anteriorly. Blue cartilage markings delineate the articular margins at the talocrural, subtalar, calcaneocuboid, and metatarsophalangeal joints. Lateral visualization of the tendo calcaneus matters because it clarifies the tendon’s relationship to the posterior calcaneus, the retrocalcaneal bursa, and the superior calcaneal tuberosity, the typical locus for insertional tendinopathy and Haglund-related impingement. It also supports teaching around rupture patterns, where midsubstance tears often occur a few centimeters proximal to the insertion, and it helps orient operative approaches for Achilles repair or debridement that must respect the nearby sural nerve and the lateral calcaneal branch territory.
