Description
Running along the anterolateral compartment of the male leg, the anterior tibial artery is shown emerging distal to the popliteal artery bifurcation and passing through the proximal opening in the interosseous membrane to reach the anterior surface of the membrane. From a lateral perspective, its course lies deep to tibialis anterior and the long digital extensors, then continues distally toward the ankle where it becomes the dorsalis pedis artery. Segmental branches consistent with the anterior tibial recurrent artery proximally and multiple muscular branches are suggested, with paired venae comitantes tracking close to the artery. A lateral view makes the relationship to the interosseous membrane and the fibula easy to read, which is exactly what you want when teaching compartment anatomy and planning exposure in the anterior leg. This is the artery at risk during anterior compartment fasciotomy and during surgical approaches to the proximal tibia and interosseous region, where misdirected dissection can stray deep and injure the vessel as it crosses from posterior to anterior. It also helps explain why distal pulses can be preserved or absent depending on whether disease affects the anterior tibial artery versus the posterior tibial and peroneal (fibular) arteries. Short and direct.
