Description
Running along the anterior compartment of the upper arm, the biceps brachii is presented in lateral profile beneath an intact cutaneous envelope, with the long head positioned more laterally and the short head lying more medially as they converge into a single fusiform belly. Proximally, the long head tendon tracks superiorly toward the shoulder, while the short head aligns toward the coracoid region, where coracobrachialis also originates and descends deep and medial to the biceps. Distally, the biceps narrows toward the cubital fossa, its tendon continuing toward the proximal radius while the muscle mass defines the familiar anterolateral contour between shoulder and elbow. Lateral surface anatomy of the biceps matters when you are correlating what you palpate with what sits deep to the skin, because the long head dominates the lateral outline and is the portion most affected by proximal tendon pathology. Pain and deformity after a long head tendon rupture often present as a distalized biceps belly (the Popeye sign) with bruising tracking down the anterolateral arm, and this side-on relationship helps explain why the contour change is so obvious even without deep dissection. It also supports teaching the course of the tendon across the shoulder and why anterior shoulder pain can be referred along the biceps groove.
