Description
Seen in lateral profile, the male mandible is rendered with partial transparency to expose the mandibular dentition from the central and lateral incisors anteriorly through the canines and premolars to the first through third molars posteriorly. Tooth crowns sit superior to their roots, which project inferiorly into the alveolar process, with the molar roots positioned closer to the ramus than the anterior single rooted teeth. Traced within the body of the mandible, the inferior alveolar nerve enters at the mandibular foramen on the medial aspect of the ramus and courses anteroinferiorly within the mandibular canal beneath the apices, running roughly parallel to the occlusal plane before continuing toward the mental region. Spatial relationships are the point here. Appreciating how close the mandibular canal lies to the roots of the molars and premolars helps explain why inferior alveolar nerve injury remains a feared complication of third molar extraction, sagittal split ramus osteotomy, and endosseous implant placement in the posterior mandible. Root morphology matters too, because periapical infection or over instrumentation during endodontic therapy can irritate the neurovascular bundle within the canal and present as paresthesia of the lower lip and chin via downstream involvement of the mental nerve. A common pitfall.
