Description
Seen from the lateral neck, the platysma forms a thin muscular sheet within the superficial fascia, lying immediately deep to the dermis and superficial veins. Its fibers ascend from the pectoral and deltoid fascia over the clavicle toward the inferior border of the mandible, blending superiorly with tissues at the lower face near the angle of the mouth. Posteriorly, the free margin approaches the anterior border of the sternocleidomastoid, leaving the parotidomasseteric region and mandibular ramus as key superior landmarks. This lateral perspective matters because the platysma defines the subcutaneous surgical plane in rhytidectomy and anterior neck rejuvenation, where dissection either preserves the muscle as a flap or intentionally crosses it to address platysmal banding. The cervical branch of the facial nerve runs on the deep surface of the platysma, while the marginal mandibular branch becomes vulnerable near the inferior mandibular border as the muscle fibers insert and decussate, a relationship surgeons keep in mind during submandibular access and parotid tail work. In trauma and neck infections, the platysma also marks a clinical threshold, penetrating injury deep to platysma often prompts evaluation for violation of deeper cervical compartments. Use this artwork in gross anatomy and head and neck modules to teach the distinction between superficial fascia, platysma, and the investing layer of deep cervical fascia, and in operative atlases to orient skin-crease incisions, subplatysmal flaps, and botulinum toxin injection planning for anterior platysmal bands.
