Description
Emerging as a thin, superficial cervical sheet, the platysma spans the anterolateral neck from the upper thoracic fascia over the clavicles and pectoral region, then ascends superiorly toward the inferior border of the mandible. Its fibers lie anterior to the sternocleidomastoid muscle bellies and superficial to the laryngeal and infrahyoid region, with the paired right and left portions approaching one another across the anterior midline. Superiorly, the muscle blends with the lower facial soft tissues near the angle and body of the mandible, creating a continuous plane between neck and lower face. Bony context appears through semi-transparent skull, mandible, clavicles, and upper ribs. Anterior orientation makes the platysma’s clinical footprint clear: it is a principal superficial landmark during subplatysmal flap elevation for neck dissection, tracheostomy exposure, and cosmetic rhytidectomy or platysmaplasty. Midline decussation and variable fiber continuity explain why anterior neck bands form with aging and why symmetry can be hard to achieve when plicating the muscle. Thin muscle. Big consequences. Teaching programs in head and neck anatomy, surface anatomy, and facial plastic surgery can use this view to clarify fascial planes, the relationship of platysma to the sternocleidomastoid borders, and how the mandible frames superior platysmal attachment.
