Description
Arising from the superficial fascia over the upper pectoral region and anterior deltoid, the platysma forms a broad, thin muscular sheet that ascends over the anterolateral neck toward the inferior border of the mandible and lower face. From this anterior perspective, its fibers lie superficial to the paired sternocleidomastoid muscles, which run obliquely from the mastoid processes inferomedially to the manubrium sterni and medial clavicles, creating a clear layered relationship. Superiorly, the mandible, maxilla, nasal bones, and orbital margins frame the lower facial attachments, while inferiorly the manubrium and upper ribs provide bony context for the platysma’s caudal origin. The midline remains comparatively thin, where right and left platysma approach across the anterior neck. Anterior visualization of the platysma matters because it is the first muscular plane encountered in many submandibular and anterior cervical approaches, and its integrity influences postoperative contouring and scar behavior. Surgeons planning a rhytidectomy, submental lipectomy, or platysmaplasty rely on this orientation to anticipate platysmal banding and decide where to divide or plicate the muscle. This view also supports teaching of the superficial cervical fascia and the vulnerable plane where superficial veins and cutaneous nerves course beneath or through the platysma.
