Description
Spanning the anterolateral neck in a male subject, the platysma is rendered as a thin superficial sheet extending from the superior thoracic fascia over the clavicle toward the inferior border of the mandible and lower facial soft tissues. Its fibers cross obliquely over the clavicular region, lying anterior to the sternocleidomastoid, and blend variably with the cervical fascia and the superficial tissues of the shoulder. Deep landmarks orient the surface anatomy: the mandible and dentition superiorly, cervical vertebrae posteriorly, and the upper ribs and scapula providing inferior and posterolateral context. A thin muscle. A big surgical consideration. Lateral presentation of the platysma clarifies the “superficial-to-deep” sequence that matters in anterior neck approaches, where the platysma is the first named muscle encountered after skin and subcutaneous tissue. That relationship underpins common teaching points in neck dissection and tracheostomy exposure, and it also explains why the transverse cervical incision is planned within relaxed skin tension lines while still respecting platysmal fiber direction to reduce banding and improve closure. The depiction of fiber intermingling near the clavicle and shoulder helps when discussing platysma flaps in head and neck reconstruction and when distinguishing platysmal edges from superficial fascia during subplatysmal elevation. Use this illustration in gross anatomy and surgical anatomy courses to teach cervical fascial planes, or in otolaryngology, plastic surgery, and general surgery publications covering subplatysmal dissection, rhytidectomy anatomy, and anterior neck incisions.
