Description
Positioned on the lateral surface of a male cerebral hemisphere, the posterior ramus of the sulcus lateralis (lateral sulcus, Sylvian fissure) arcs superiorly and posteriorly as it separates the temporal lobe inferiorly from the frontal and parietal opercula superiorly. Adjacent gyri along its superior margin include the inferior frontal gyrus anteriorly and the precentral and postcentral regions more posteriorly, while the superior temporal gyrus runs parallel along the inferior bank. Posteriorly, the sulcus lateralis approaches the supramarginal gyrus as the parietal lobe wraps around the terminal end of the fissure. Cerebellum and brainstem sit posteroinferiorly as background landmarks. This lateral perspective matters because the posterior Sylvian region frames perisylvian cortex that anchors language networks, and it is also where the middle cerebral artery territory dominates cortical perfusion. Ischemic stroke involving MCA superior division branches often affects frontal and parietal opercular areas along the superior bank, while inferior division infarcts track the temporal operculum and superior temporal gyrus, a pattern that maps cleanly onto the anatomy of the posterior ramus. Opercular retraction during approaches to the insula also references this fissure as a surface corridor. A clean landmark.
