Description
Positioned in lateral profile, the maxillary first molar is isolated in transparent blue against the adjacent maxillary dentition, with the crown occlusal surface oriented superiorly and the cervical line tapering into the root trunk inferiorly. Three roots are evident, a broad palatal root projecting medially and two buccal roots (mesiobuccal and distobuccal) lying more laterally, diverging as they course apically within the alveolar process of the maxilla. Overlying this posterior maxilla, the floor of the maxillary sinus sits superior to the molar apices, separated by a variable thickness of bone. Root form matters here. The mesiobuccal root often carries a second canal (MB2), a common reason for persistent apical periodontitis after endodontic therapy when it is missed or inadequately instrumented. Just as clinically relevant, the close relationship between posterior maxillary molar apices and the maxillary sinus underlies odontogenic maxillary sinusitis and the potential for oroantral communication after extraction, apicoectomy, or implant site preparation, and this lateral perspective makes the apical proximity and root divergence easier to appreciate than a purely occlusal view. Small distance, big consequences. Use this illustration in preclinical dental anatomy and endodontics teaching to discuss root curvature, canal prediction, and access strategy, or in oral and maxillofacial surgery and ENT materials covering sinus floor anatomy and dental sources of sinus disease. It also fits well in patient-facing education on why upper molar procedures can involve the sinus and why CBCT assessment may be requested.
