Description
Seen in lateral profile on a male skull, the zygomaticus major originates from the lateral surface of the zygomatic bone, just anterior to the zygomaticotemporal suture, then courses inferomedially toward the modiolus at the angle of the mouth. Its fascicles pass superficial to the maxilla and buccinator region, placing the muscle anterior to the mandibular ramus and distinct from the masseter on the lateral jaw. Cranial sutures, the zygomatic arch, maxilla, mandible with dentition, and the nasal bones framing the nasal cavity provide fixed bony landmarks. Inferiorly, the articulated cervical vertebrae sit posterior to the pharyngeal space, with intervertebral discs indicated. Appreciating the line of pull of zygomaticus major matters when you are teaching facial expression mechanics or planning procedures that traverse the cheek. Its insertion near the oral commissure explains the superolateral displacement of the mouth corner during smiling, and it helps distinguish a true smile from movements dominated by risorius or depressor anguli oris.
