Description
Arising from the lateral surface of the zygomatic bone (malar bone), the zygomaticus major is isolated and color-highlighted as it courses inferomedially toward the modiolus at the oral commissure. A lateral skull and upper cervical spine provide bony context, including the zygomatic arch, maxilla, mandible with dentition, and cranial sutures, while the nasal cartilages are contrasted in blue along the external nose. The muscle belly and its fascicles are oriented superficial to the maxilla and anterior to the masseter region, then terminate at the corner of the mouth where fibers blend with the orbicularis oris and other perioral muscles. Spatial relationships read cleanly. Facial expression anatomy lives or dies by attachments, and this lateral perspective makes the zygomatic origin and commissural insertion easy to teach without guessing where the smile vector comes from. Weakness of this muscle on one side contributes to facial asymmetry in peripheral facial nerve (CN VII) palsy, and the line of pull toward the modiolus helps clinicians distinguish true paresis from compensatory overactivity of the contralateral mimetic musculature. Reconstructive and aesthetic planning also depends on it, since facelift dissection planes and midface procedures must respect superficial mimetic muscles and their tethering to the dermis around the nasolabial region.
