The rib’s head sits medial and posterior, while the tubercle and adjacent neck lie lateral to the vertebral column, framed by the thoracic vertebrae and the posterior rib cage. A medial perspective on the first rib clarifies why it is a high-stakes anatomic corridor: the subclavian vessels and lower roots of the brachial plexus occupy fixed relationships to the scalene attachment, so small changes in rib position or scalene tone can translate into neurovascular compression.
