Description
Seen from a medial thoracic perspective, the right third rib (costa III) arcs anterolaterally from its posterior vertebral attachment toward the costal cartilages, with the head and neck closely apposed to the lateral aspect of the upper thoracic vertebral bodies. The tubercle lies posterolateral to the costovertebral junction, aligning with the transverse process region, while the shaft’s gentle curvature carries it anteriorly and inferiorly. Along the inferior internal border of the rib, the costal groove (sulcus costae) is visible as a longitudinal depression, positioned inferior to the thicker superior margin. Posteriorly, adjacent thoracic vertebrae and intervertebral discs provide orientation, and the thoracic kyphosis is apparent. A medial view that emphasizes the costal groove matters because it is the bony landmark that shelters the intercostal neurovascular bundle in the costal groove between the internal and innermost intercostal muscles. That relationship drives technique: pleural aspiration and chest drain placement are performed just superior to a rib to avoid the intercostal vein, artery, and nerve that run along its inferior border. Rib III also helps teach rib counting from the sternal angle (rib II) and the higher interspaces targeted for certain thoracic procedures, where small errors in level selection can lead to ineffective drainage or iatrogenic injury.
