Description
Seen from the medial aspect, the right ninth rib (costa IX) curves anteriorly from its posterior (vertebral) end toward the costal margin, with the head and neck directed medially toward the thoracic vertebral bodies and the tubercle aligned for articulation with the transverse process at T9. The shaft (corpus costae) shows its angle just lateral to the tubercle, then sweeps anteroinferiorly toward the costochondral junction. Anteriorly, the ninth costal cartilage continues superiorly to blend with the cartilage of rib VIII, forming part of the arch of the costal margin rather than attaching directly to the sternum. Adjacent thoracic vertebrae and intervertebral discs provide a midline reference, with the rib lying lateral to the vertebral column in anatomical position. A medial perspective is the clearest way to teach the costovertebral and costotransverse joints, including where the ninth rib head meets the demifacets of T8 and T9 and where the tubercle meets the transverse process. That relationship matters during thoracotomy and rib resection, because dissection near the posterior rib risks the intercostal neurovascular bundle running in the costal groove on the inferior internal surface, and pain patterns often track the ninth intercostal nerve. Lower rib and costal cartilage mechanics also explain why costal margin tenderness can mimic upper abdominal pathology, and why fractures of ribs 9 to 12 raise concern for splenic, hepatic, or renal injury depending on side and level.
