Description
Seen from a medial-lateral profile, the male vertebral column is arranged into cervical, thoracic, lumbar, sacral, and coccygeal regions, with intervertebral discs highlighted between adjacent vertebral bodies from C2 to S1. Disc tissue sits anterior to the vertebral canal and posterior to the anterior longitudinal ligament, while the posterior disc margin aligns with the posterior longitudinal ligament along the posterior aspect of the vertebral bodies. Cervical lordosis, thoracic kyphosis, and lumbar lordosis are preserved, and the thoracic ribs articulate posteriorly with the thoracic vertebrae, curving anterolaterally to frame the hemithorax. A clean sagittal silhouette. Emphasis on the posterior disc contour matters because posterolateral disc herniation most often tracks toward the intervertebral foramen, where it can compress the exiting spinal nerve root (for example, an L4-L5 disc affecting the L5 root, or an L5-S1 disc affecting the S1 root). The profile also clarifies why thoracic disc disease presents differently: the rib cage stabilizes the thoracic segment, limiting flexion and rotation compared with the lumbar spine and changing load distribution across the annulus fibrosus. Clinically, these relationships guide interpretation of sagittal MRI and planning of approaches such as lumbar microdiscectomy or anterior cervical discectomy and fusion (ACDF), where disc level and curvature dictate trajectory.
