Description
Running obliquely down the anteromedial forearm, the flexor carpi radialis (FCR) muscle belly sits superficial in the flexor compartment, arising from the common flexor origin on the medial epicondyle of the humerus and narrowing into a long tendon as it approaches the wrist. Medially, the flexor carpi ulnaris forms the ulnar border of the superficial flexor mass, while the palmaris longus (when present) typically lies just ulnar to the FCR tendon. Deep to this plane, the flexor digitorum superficialis contributes bulk in the mid-forearm, and the radius lies laterally while the ulna remains medial; at the distal forearm the FCR tendon tracks toward the bases of the second and third metacarpals. Skin remains intact. Clinically, the distal FCR tendon is a dependable surface landmark because the radial artery runs immediately lateral to it at the wrist, a relationship exploited for arterial cannulation and palpation during vascular assessment. Hand surgeons also use the interval along the FCR tendon for the volar approach to the distal radius and radiocarpal joint, with the palmar cutaneous branch of the median nerve and the superficial branch of the radial nerve both at risk from retraction or poorly placed incisions. Overuse can produce FCR tendinopathy where the tendon courses through its fibro-osseous canal at the trapezium, presenting as volar-radial wrist pain that can mimic first carpometacarpal osteoarthritis.
