Description
Running along the anterior compartment of the forearm, the flexor carpi radialis (FCR) is highlighted as a fusiform muscle belly arising from the medial epicondyle via the common flexor tendon and tapering into a long distal tendon. From a palmar (anterior) perspective, the FCR lies superficial and slightly lateral to palmaris longus when present, and medial to brachioradialis and the radial wrist extensors. Distally, its tendon courses toward the wrist on the radial side of midline, passing deep to the flexor retinaculum in its own fibro-osseous canal before inserting primarily on the base of the second metacarpal (often with a slip to the third). Key neighbors sit close. The radial artery runs just lateral to the tendon at the wrist, while the median nerve tracks more centrally in the forearm. This anterior view matters because the FCR tendon is a practical surface and surgical landmark for the volar wrist. Hand surgeons commonly use the interval along the FCR sheath during the volar approach to the distal radius for open reduction and internal fixation, and the relationship to the radial artery explains both safe dissection planes and where vascular injury occurs when the tendon is misidentified. Clinically, FCR tendinopathy and tenosynovitis present as pain along the radial volar wrist and can be confused with scaphoid or first carpometacarpal pathology, so seeing the tendon’s straight course to the second metacarpal helps tighten a differential diagnosis.
